All forms and paperwork needed to participate in or contribute to LMHS Mini-THON may be found by clicking the links below.

For new volunteers, all three of the following requirements must be met prior to beginning service as a volunteer:

The disclosure statement is needed if you HAVE lived in Pennsylvania for the last 10 years.

The FBI Fingerprint Check is needed if you HAVE NOT lived in Pennsylvania for the last 10 years.

In selecting the Service code for the FBI fingerprint check (first prompt), the candidate should use the following: PDE - School District - 1KG6XN. Please provide the District the UEID # from your receipt once you get fingerprinted so we may access the results online.

Please submit the required documents in one packet to the address as listed below.

Once you have been approved as a volunteer, a yellow Volunteer Card will be sent to you in the mail.  Please show this card to obtain admittance to a school.
